NHS Care Records Service

The NHS in England is introducing a national Summary Care Records as part of the NHS Care Records Service. This is to improve the safety and quality of patient care.

It will give healthcare staff faster, easier access to reliable information about you to help with your treatment.

Today, all NHS organisations where you receive care keep records about you. They can usually only share information from your records by letter, e-mail, fax or phone. At times, this can be slow and sometimes things get lost on the way.

Now there will be quicker ways to get important information to NHS staff treating you, including in an emergency.

Summary Care Records

Over time, you will have a Summary Care Record (SCR) created, which will be available to people providing you with care anywhere in England.

At first, your SCR will contain important information about your health, such as details of any allergies, your current prescriptions and whether you have had any bad reactions to medicines.

After that, each time you use any NHS health service, we may add details about any health problems, summaries of your care and the professionals treating you to your SCR.

As we add new information to your record, you can discuss what is being added and how sensitive information is handled.

Every time a clinician need to look at information in your Summary Care Record they will ask your permission. When this is not possible, for example in an emergency when you are unconscious, they will tell you later.

National Summary Care Records will gradually be introduced across the Central and Eastern Cheshire PCT locality. All patients over the age of 16, registered to a GP practice in the area, have already received a letter about SCR, informing them of their options. If you are happy for us to create a SCR for you, you do not need to do anything. You can also choose not to have a SCR created if you wish.
