Carers
What is a Carer?
A Carer is anyone who provides help and support to a relative, partner or friend who cannot manage because of disability, illness or frailty.
Does your GP surgery know you are a carer?
Why is it important for my GP surgery to know that I am a carer?
Some GP surgeries in Cheshire record the names of Carers. This information helps your surgery to:
- Make sure you recieve information about the help and support that is available
- Refer you for a 'Carers Assessment' if you would like one
- Improve services for you as a Carer.
Help and support your surgery may be able to give you
- Access to a Carers information pack and other information
- Refer you to Cheshire Carers Centre and other helpful organisations which support Carers and families
- Give you appointment times to fit in with your caring role
- A health check if caring for someone is affecting your health
- A flu jab
- Information about a Carers Assessment - so that your needs can be looked at.
How do I let my GP Surgery know I am a carer?
Pick up a leaflet at your surgery, fill in the form on the reverse and hand it to the receptionist.

Draft Joint Strategy for Carers
Cheshire East Carers Strategy forms the basis for developing and delivering services for carers. It has been produced through partnership work between the Primary Care Trust (CECPCT), The Adults, Community Health and Wellbeing Department and some carers, to provide the framework for members of the Interagency Carers Strategy Implementation Group, statutory and third sector organisations and individuals to work together to improve the quality of life of carers in the Borough. See the Draft Joint Strategy and make your views count.
